Summary

The pharmacist is responsible for providing comprehensive medication managementin a fast-paced, high-acuity emergency department (ED). As a member of the multidisciplinary healthcare team, the clinical pharmacist promotes an environment of safe and effective medication utilization while providing services to meet the needs of the patient.

Responsibilities

  • Works collaboratively with the ED healthcare team to develop and optimize patient medication therapy.
  • Provides comprehensive pharmacy medication management review of ED medication orders including during medical emergencies, code resuscitation efforts, and high-risk procedures.
  • Completes medication order verification reviews for both ED medication orders and discharge medication orders.
  • Provides knowledge of pharmacotherapy practices for pediatric emergency medicine including medication indication, dose, adverse effects, and monitoring to physicians, nurses, and patients/families.
  • Participate in bedside response for all patients needing immediate intervention including critically ill patients, patients with high risk for sepsis, behavioral emergencies, seizures, and more.
  • Facilitates the preparation and labeling of medications to meet the immediate needs of critically ill patients.
  • Provides discharge counseling to patients and caregivers for home emergency rescue medications.
  • Adapts to changes in work environment to provide effective patient care.
  • Participates in quality improvement initiatives and attends hospital committee meetings as needed.
  • Maintains current knowledge in area of expertise including participating in continuing education programs, in-services, and literature review to enhance professional growth.
  • Contributes to pharmacy and medical literature and participates in research where applicable.
  • Provides education to pharmacy staff, nurses, and providers including precepting pharmacy residents and students.
  • Participates in the development, implementation, and ongoing review of Epic medication orders and medication order set builds as needed.
  • Maintains responsibility and accountability for successfully completing tasks or projects.
  • Performs other duties and special projects as required.

This will be a full time, overnight position in the emergency department that follows a 7 on 7 off schedule pattern.

Qualifications

Required

  • Doctor of Pharmacy from an accredited school of pharmacy or equivalent degree
  • Pharmacy licensure in the state of Texas
  • One year of hospital experience
  • 20-hour ACPE accredited IV sterile compounding training course required within 90 days

Preferred

  • Completion of PGY1 ASHP-accredited pharmacy residency or equivalent experience (3 years of hospital experience); pediatric-focused PGY1 or pediatric hospital experience preferred
  • Completion of PGY2 ASHP-accredited pharmacy residency in pediatrics, critical care, or emergency medicine
  • Board of Pharmacy Specialties Certification

Certifications

Required within 90 days of hire

  • Basic Life Support (BLS) for Healthcare Providers
  • Pediatric Advanced Life Support (PALS)
  • Advanced Cardiovascular Life Support (ACLS)
