For individuals transitioning from closed custody correctional facilities, Cherry Health provides Person Centered Reentry Services to ease the transition back into the community. Services are available at two locations: our Community Treatment Center (CTC) in Detroit and Community Alternatives Program (CAP) in Grand Rapids. Services include 24-hour residential treatment in a structured environment with individual and group counseling services. Some participants come to us directly from a correctional facility, while others come as part of a diversion program to keep them out of prison. Together, we work to address health issues (including behavioral health issues) and other reentry needs i.e., employment, education, housing, and fostering connections to help justice system involved persons successfully reintegrate back into their respective communities.

Job Description Summary:
The Case Manager - Reentry is responsible for managing and coordinating clinical activities for residents, including assessments, treatment planning, counseling, and discharge planning. This position ensures that residents receive the appropriate clinical care and services while adhering to established policies and procedures. The Case Manager works closely with the Clinical Manager to maintain continuity of care and facilitate clinical case reviews and group counseling.

This is a full time, benefit eligible position typically working day shift hours Monday - Friday onsite at our Community Treatment Center in Detroit.

Responsibilities:

  • Case management responsibilities on assigned clients to include: biopsychosocial assessments, treatment plans, individual counseling, group counseling, case conference presentation, progress reports to referral sources, facilitate outside appointments, aftercare development, discharge planning and paperwork.
  • Consult with the Clinical Supervisor/Manager in developing and maintaining the continuity of treatment and resident activities within the program.
  • Coordinate and monitor resident activities within the program treatment model.
  • Participate in Program Review Team meetings to discuss trends and dynamics of the treatment population as well as participate in clinical case reviews.
  • Facilitate psychoeducational groups as assigned.
  • Maintain therapeutic environment.
  • Provide disciplinary action on incidents of clients in consultation with Clinical Supervisor, Manager.                                                          
  • Maintain records of resident progress in a timely manner.
  • Other duties as assigned.

Qualifications:

  • Education: Bachelor’s degree in social, human services, criminal justice or behavioral science program required.  LMSW or Master’s degree preferred.  MCBAP certification desired.  Excellent driving record and current valid driver’s license.
  • Experience: Minimum of one year of experience in human services, corrections, or community-based services required. Three years of social work experience, including at least two years of group and casework experience, preferred. Work experience may be substituted for academic studies—one year of work experience in a related field may replace one year of academic education. A total of five years of experience may be substituted for education requirements.

    • Knowledge of correctional criminogenic needs and the criminal justice system.
    • Ability to develop and implement treatment plans and case conferences.
    • Experience with vocational and career counseling.
    • Understanding group dynamics and group processes.
    • Ability to utilize ancillary services and knowledge of available resources.
    • Strong statistical record keeping and data compilation skills.

Physical Requirements:

  • Possible exposure to patients/clients with illnesses and/or infections.
  • Frequent telephone and computer use, requiring manual dexterity for writing and computer work.
  • Sitting, standing, or walking for extended periods.
  • Specific vision abilities may include close vision, distance vision, and the ability to adjust focus.
  • May require bending, squatting, kneeling, and lifting up to 50 pounds.
  • Use of Personal Protective Equipment (PPE) as required.
