As a Connecting Communities Recovery Coordinator, you will develop strong partnerships and joint working within the prison and beyond into local communities to actively improve services, seek to reduce reoffending to enhance crime free communities and improve health and well-being outcomes for service users in our care.

 

You will engage, promote, and deliver mentor training within the prisons, looking for opportunities for this to continue in community services upon release.

 

Where: HMP Liverpool 

Full Time Hours: 37.5 hours per week 

Full Time Salary Range: £27,861.26 - £32,002.35*

Contract Type: 12 month fixed term contract from start date

 

*Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role: 

  • Provide interventions both group and 1:1 for hard to reach service users directed at through the gate provision within the secure estate. 
  • Work as part of a prison Team with outreach elements to community engagement.
  • Give advice & interventions for harm reduction, blood borne viruses and overdose prevention advice and naloxone provision.
  • Provide assessment and interventions in a prison setting including full release planning for continuity of care on release. 
  • Support community agencies to gain access to service users pre-release, set up gate pick ups and through the gate provision.
  • Engage all service users who may benefit from a substance misuse intervention. 
  • Create a confidential, safe and secure environment for service users, encouraging engagement with community services on release.

 

About you:

  • Have a strong understanding of substance misuse issues and experience of working within a similar field 
  • Experience of working with groups/pods and managing group dynamics, such as conflict or challenging behaviour. 
  • Be an excellent communicator, both verbal and written with good IT skills 
  • Proactive and able to work on own initiative with excellent time management and prioritising skills 
  • Have a good working knowledge of mental health interventions, services, and good practice 
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders 
  • Ability to manage change successfully in a way that prioritises the needs of service users 
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards 
  • Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
