MHM Services, a Centurion company, is proud to be the provider of mental health services to the California Department of State Hospitals Conditional Release Program (CONREP). The CONREP program is a statewide system that provides standardized intensive community outpatient mental health services for judicially committed individuals and mentally disordered offenders.
We are currently seeking a full-time Socialization Coordinator/Case Manager to join our team at our Contra Costa CONREP program located in Concord, California.
Schedule: Monday through Friday; 8:00am to 4:30pm
The Socialization Coordinator/Case Manager provides community-based services and monitoring to clients that are judicially committed in addition to mentally disordered offenders. The Socialization Coordinator is responsible for providing individualized recreational and socialization support and services as well as psychoeducation to address coping, life, job, self-care, and social skills.

Qualifications
Minimum of one (1) year of mental health experience; community-based mental health experience preferred
Must possess and maintain a valid state driver's license with good driving record
Ability to pass drug screen and criminal background check
