Position Summary:

Under the direction of the Director of Clinical Services, employee completes the MDS-RCA, updates resident care plans, assists with admissions of new residents, discharges of residents, performs nursing chart audits semi-annually, and is a resource person for the charge nurses. Responsible for the maintenance and update of resident individual service plans. May perform other duties as required.

Essential Duties:

• Completes the scheduled MDS-RCA assessment using the state specific form and guidelines.• Data inputs MDS-RCA assessment into Hi-tech computer system.• Prints and signs completed form. Creates and reviews resident individual service plan on admission, semiannually and as indicated by a significant change in the resident’s status.• Create and submit a monthly submission containing RCAs for the month to the state of Maine.• Schedule residents for Residential Care Assessment completion twice yearly, and as indicated.• Schedule residents for Individual Service Plan meetings on bi-annual basis.• Forward date of meetings to the Social Service Coordinator so that invitations may be sent to family members.• Ensures that all reports, data sheets, professional assessments, etc., are updated, signed and dated by appropriate personnel.• Assist with admission of a resident to the facility. • Monitor chart for completion of admission paperwork. • Assist the charge nurse as needed with admission paperwork.• Assist with all residents admitted to the facility. • Ensure required paperwork has been completed upon or after admission. • Required paperwork as follows: PNMI, Dr. orders, signed H & P, face sheet, admission note, Nursing assessment, release of medical information, house rules, smoking rules, admission height and weight and all other required paperwork.• Assist Director of Clinical Services in ensuring safe discharges of residents.• Ensure required paperwork has been completed.• Complete a Bi-annual chart audit of resident’s record to ensure that all records are maintained at state standard for medical records.• Is available for charge nurses as a resource person to observe/assess a resident or advise nurse on medical concerns.• Functions as charge nurse as needed.• May be on call person when Director of Clinical Services is away or on vacation.• Perform special assignments assigned by the Director of Clinical Services.• Provides weekly/monthly submission reports to Administrator and Director of Clinical Services.• Responsible for Resident Assessment documentation training, periodic reviews and follow up of residential aide documentation.• Customer Service: Interacts with all individuals in a consistent manner, providing attention, support, and assistance to foster an environment of exceptional personal service

 

 

Education and Experience:

• High School Diploma or equivalent with some business college courses preferred.• Minimum of 1 year bookkeeping experience.• Must have well-developed organizational skills.• Must be proficient in Microsoft Word and Excel. • Basic computer knowledge.• Must be able to maintain a calm demeanor when dealing with team members, residents and visitors.• Ability to work independently on a variety of tasks.• Must have 2 years of experience in Residential Care• Must have working knowledge of Residential Care State regulations.• Must have excellent organizational skills and must be able to focus on and pay close attention to detail.• Must be tolerant, patient and treat residents with dignity and respect.

• Able to effectively communicate in English, both verbally and in writing.• Strong written and verbal skills.• Ability to engage patients and team members utilizing the CMH Experience Standards
