Family Medicine Advanced Practice Provider

CentraCare is hiring a full-time Family Medicine Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with our team at the Melrose clinic.

Responsibilities
As a provider developing your own patient panel, you will independently see patients of all ages for acute and chronic care needs. Our CentraCare – Melrose clinic providers diagnose and treat short-term illnesses; provide preventive care including routine checkups, health-risk assessments, immunizations and screening tests; offer personalized counseling on maintaining a healthy lifestyle; and manage and coordinate care for those with chronic illnesses. The Melrose clinic has access to lab and imaging services during regular clinic as well as extended hours.

Schedule

.9 – 1.0 FTE
Option of a four-day work week
Limited weekend work consisting of Saturday morning clinic rotation 1:6
No call
No holidays

Requirements

To be eligible for this position you must meet the following requirements:

Certified as a Nurse Practitioner or hold a current Minnesota Physician Assistant Licensure and be eligible for Minnesota licensure.
All applicants must have a current American Heart Association BLS Healthcare Provider card and Drug Enforcement Agency (DEA) registration number prior to receiving clinical privileges.

It is preferred that applicants have relevant practicing provider experience in one of the following areas:

Family Medicine
Emergency Medicine
Urgent Care

Pay and Benefits

Starting pay begins at $109,241.60 per year; exact wage determined by years of related experience
Salary range: $109,241.60 - $124,904 per year
Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate

Dedicated primary clinic nurse (CMA, LPN or RN) with support of float nursing
Co-located workstations for provider/nursing with an additional office
Opportunity for voice-recognition software through DAX Co-pilot
Team support including RN nurse lead visits, diabetic education, health care home for complex patient care, legal aide, health care navigator for uninsured/underinsured
Community paramedic for patient engagement and home visits
Prescription support programs for uninsured/underinsured
In-room schedulers to promote continuity (follow-up appointments made for patients before they leave the room)
CentraCare Foundation supported Reach Out & Read Program
Mental Health support including Integrated Behavioral Health with often same-day video consultation and Psychiatry chart review for medication management
E-consults for specialty care to allow for PCP management without the wait for a formal consult

Hospital

State-of-the-art Critical Access Hospital built in 2017 with significant provider and community input

Services include:

Telestroke and STEMI activation with St. Cloud Hospital
Advanced imaging with 5 days/week MRI availability, 3D Mammography
Nitrous oxide for obstetrical and ED patients
Lactation consultants
Rehabilitation Services including Multidisciplinary Cardiac Rehabilitation and Pulmonary Rehabilitation
Transitional Care Team including a nursing supervisor, PT, OT, speech therapy and social worker
Outpatient Services including chemotherapy and IV infusions of fluids and medications
