JOB DESCRIPTION:

Position Summary
Provides therapeutic and case management interventions to support recovery, stability, and resiliency for individuals with mental illness, many of whom have co-occurring substance use disorders, within a community-based Assertive Community Treatment (ACT) model. Conducts ongoing assessments of client needs and functioning, develops and implements individualized, person-centered treatment plans, and maintains required clinical documentation in accordance with program and regulatory standards. Collaborates closely with a multidisciplinary team to promote engagement, recovery, and long-term outcomes.

Essential Duties and Responsibilities

  • Provides individual and group therapy services to clients, including individuals with serious mental illness and co-occurring substance use disorders, in both office-based and community-based settings.

  • Collaborates with clients, families, and the multidisciplinary team to develop and maintain person-centered, family-centered, and service-driven individualized service plans. Monitors client progress, participates in service plan reviews, updates plans as clinically indicated, and submits plans for medical review when required.

  • Completes intake and admission processes for clients entering services, including explaining available services, gathering required documentation, ensuring client understanding of forms, obtaining appropriate signatures and consents, and submitting materials for processing. Conducts clinical assessments and prepares assessment summaries as assigned.

  • Supports clients in accessing internal and external community resources in coordination with Integrated Healthcare Services. Maintains appropriate communication with referral sources, provides updates on client progress and needs (with proper releases), and participates in ongoing consultation as appropriate.

  • Assists clients with discharge planning and transition services, including coordination of follow-up care and ongoing supports to promote continuity of care and long-term stability.

$48,300.00--$67,700.00
