
Collaborating Physician: Dr. Moskowitz
Location: Catholic Health Ambulatory Care at Merrick
Catholic Health is seeking a Nurse Practitioner to provide primary care services in an ambulatory setting at Catholic Health Ambulatory Care at Merrick. The NP will support a team of experienced Family Medicine physicians and practice under the supervision and collaboration of Dr. Moskowitz.
The NP is an integral member of the CHS Physician Enterprise and provides patient care under physician supervision, credentialed through the Medical Staff process.
Dr. Moskowitz (Collaborating Physician)
Board-certified Family Medicine physician providing primary care at Bellmore Family Practice. Dr. Moskowitz completed his medical education at the New York College of Osteopathic Medicine and postgraduate training at Good Samaritan Hospital, where he served as Chief Resident in Family Medicine. He is on staff at Mercy Hospital and St. Francis Hospital and serves as Chair of the Primary Care Service Line in Nassau County for Catholic Health.
Dr. Abdul Siddiqui, DO
Board-certified Family Medicine physician with 6–10 years of clinical experience. Dr. Siddiqui completed his residency at North Shore Plainview Hospital and has expertise in managing chronic conditions including hypertension and asthma.
Perform patient histories, physical exams, and assessments
Develop, document, and communicate plans of care
Order and interpret labs, medications, and diagnostic tests
Provide follow-up care and patient education
Collaborate with physicians and care teams
Maintain accurate EMR documentation
Graduate of an accredited Nurse Practitioner program
Active NY State RN/NP license
AANP or ANCC certification
Minimum 2 years NP experience in ambulatory or physician practice preferred
