General Responsibilties

  1. Operate as a member of the program team.
  2. Conduct comprehensive needs assessments and coordinate services focused on obtaining permanent housing for all clients.
  3. Conduct all case management with a Harm Reduction, Client Centered philosophy.
  4. Monitor ongoing client services and advocate as needed.
  5. Provide case management services in office or field-based settings.
  6. Handle complex cases and conduct follow-up as required.
  7. Maintain confidential case records and complete required documentation.
  8. Maintain cooperative relationships with service providers and community organizations.
  9. Other duties as assigned within the guidelines of this position.

Requirements


  1. Bachelor's degree in social work, psychology, or related field with one year of case management experience required; high school diploma with five years of related experience may substitute; Master's degree preferred.
  2. Experience with program-specific populations preferred.
  3. Experience working with low-income and homeless individuals.
  4. Availability to work flexible days and hours.
  5. Ability to work independently and respond quickly in times of crisis.
  6. Ability to relate well to individuals with disabilities and older individuals.
  7. Ability to handle complex cases and problem solve.
  8. Ability to work effectively with diverse ethnic, racial, and social backgrounds.
  9. Proficiency in Microsoft Office applications.
  10. Strong verbal and written communication skills; valid driver's license, reliable transportation, and proof of liability insurance.
