General Responsibilties
- Operate as a member of the program team.
- Conduct comprehensive needs assessments and coordinate services focused on obtaining permanent housing for all clients.
- Conduct all case management with a Harm Reduction, Client Centered philosophy.
- Monitor ongoing client services and advocate as needed.
- Provide case management services in office or field-based settings.
- Handle complex cases and conduct follow-up as required.
- Maintain confidential case records and complete required documentation.
- Maintain cooperative relationships with service providers and community organizations.
- Other duties as assigned within the guidelines of this position.

Requirements
- Bachelor's degree in social work, psychology, or related field with one year of case management experience required; high school diploma with five years of related experience may substitute; Master's degree preferred.
- Experience with program-specific populations preferred.
- Experience working with low-income and homeless individuals.
- Availability to work flexible days and hours.
- Ability to work independently and respond quickly in times of crisis.
- Ability to relate well to individuals with disabilities and older individuals.
- Ability to handle complex cases and problem solve.
- Ability to work effectively with diverse ethnic, racial, and social backgrounds.
- Proficiency in Microsoft Office applications.
- Strong verbal and written communication skills; valid driver's license, reliable transportation, and proof of liability insurance.
