Home and Community Based Services provides community habilitation services to individuals who are diagnosed with a Developmental Disability. Community Habilitation provides skill training in activities of daily living, academics, socialization and community inclusion. The program also provides family support by assisting families in applying for Medicaid and accessing other needed services.

STATEMENT OF THE JOB

In furtherance of an integrated approach to the delivery of person-centered services, the Habilitation Coordinator, under the guidance of the Unit Supervisor will be responsible for the development and supervision of the new staff action plans in collaboration with the life plan and all providers involved in each individuals life. 

  • To attend Life Plan meetings, to develop Staff Action Plans as to maintain documentation.
  • Work with the Community Habilitation Skill Developers on billing together with the supervisors.
  • Complete intake process with HCBS services and outreach.
  • Complete Annual Direct Support Plan evaluations.
  • Complete case notes which documents an individual’s progress, including his/her accomplishments and the prevention of regression since the last review.
  • Maintain charts with current documentation such as annual Level of Care, Semi-Annual Life Plans, Community Habilitation Plan/Staff Action Plans, Addendums, documents from other providers, liability form.
  • Will develop and implement a consumer interest tool to assist in developing meaningful and functional activities for programming.
  • Ensure that staff assist individuals in identifying the formal and informal supports available in his/her community, family and social network and works with individuals to develop and sustain relationships with friends, family and community members.
  • Ensure that systems meet regulatory compliance.
  • Ensure that individuals with personal care and physical needs are addressed appropriately.
  • Facilitate Individual services and activities that are consistent with valued outcomes of independence, individualization, inclusion and productivity.
  • Report and document all incidents in accordance with Agency guidelines and OPWDD.
  • Making referrals for the needs of the consumer/family including basic needs such as food, clothing, and shelter.

 

QUALIFICATIONS

  • Bachelor’s degree required.
  • 2 years of experience with MR/DD preferred.
  • Knowledge of Home and Community Based Waiver, and the ability to keep up to date pertaining to such knowledge.
  • Understanding of Person Centered Planning.
  • Reasonable flexibility in work hours and ability to respond to the scheduling needs of the individual and or program training.
  • Successful completion and proficiency of SCIP Training, and CPR/First Aid.
  • Valid Driver’s License preferred.
  • Re-ensure tuberculosis clearance annually (PPD/X-Ray).
  • Must be able to frequently lift up to 30 lbs.
  • Must be able to sit or stand for more than 4 hours at a time.
  • Frequent kneeling, bending, reaching, and climbing stairs.
