About this role

Job Summary: Facilitates and conducts all intake and enrollment services, from initial contact with potential participants to enrollment into the program including but not limited to prescreening of interested parties, scheduling of appointments, managing enrollment calendar, touring facilities, home assessments, yearly re-certifications,detailed tracking of Intake/Enrollment progresses, completing and submitting of Medicaid & MDS applications, and ensuring all State required documents are complete, accurate, and submitted timely. Processes all Disenrollment requests. Completes all monthly reports utilizing GenTran, Marx, and ERPT, as well as internal reports with enrollment updates. Accurately enters data into TruChart and updates any status changes. Informs IDT of potential participants. Keeps timely tracking sheet of all referrals and status. Assist with outreach and marketing efforts including but not limited to participation and set up of potential health fairs, expositions and Sr. Living presentations and other events.

 

Duties/Responsibilities

Essential Job Functions:

  • Receives and responds to all Intake & Enrollment calls and emails.
  • Evaluates each potential participant using the Prescreening Questionnaire and documents outcomes
  • Enters prospective participants into TruChart system
  • Schedules potential participant for LOCET, Initial MDS Assessments, Reassessments, and Medicaid Application.
  • Works with Enrollment Team to coordinate all intake and enrollment functions and processes.
  • Assists potential participants with getting LOCET, MDS, Medicaid applications, and Enrollment agreement completed.
  • Conducts intake visit and enrollment agreement meetings.
  • Assists participants with Medicaid renewal processing for current PACE participants.
  • Facilitates communication and expectations with potential participant and caregivers regarding any and all paperwork needed prior to Initial MDS Assessment and Medicaid application (such as bank statements, list of medications, etc).
  • Conducts tour of Center, possibly both locations based on potential participant's residence, and provides information about services provided.
  • Assures that all Medicaid applications, enrollments and disenrollments, and Medicaid requests for information are processed timely.
  • Ensures that LOCET, MDS, and Medicaid documentation is accurate, complete, processed timely and sent to State by the cutoff date.
  • Confirms activation of leads as well as newly enrolled participants in TruChart and disenrollments.
  • Confirms receipt of all State approval of enrollments and disenrollment's.
  • Responds to CMS requests for daily transaction returns, completes Enrollment validation and corrects CMS system errors
  • Coordinates with the Enrollment Team and Director of Operations regarding any issues or delays in enrollment process.
  • Keeps up to date and current Enrollment Tracking Sheet to track enrollment process
  • Updates prospective participants in TruChart system as needed
  • Completes monthly reports in GenTran, Marx, and ERPT.
  • Checks daily transactions and downloads from GenTran, Marx, and ERPT.
  • Participates in team meetings and strategy planning as necessary and makes recommendations regarding projects and process improvements.
  • Conducts marketing and outreach efforts to ensure continual flow of new enrollees.
  • Assists with training new staff as needed
  • Performs other duties as assigned

 

Working Conditions:

  • Excellent to poor lighting in working environment.
  • Contact with patients under wide variety of environments and circumstances; i.e., irrational, hostile grieving, actively dying.
  • Subject to frequent interruptions and distractions within the work environment.
  • Probability for unexpected emergency or crisis situation to occur.

 

Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Sitting, Standing, Walking, Talking, sitting at a desk, and working on a computer. Must be able to lift fifteen pounds at times. Physical Demands: Sedentary physical activity performing non-strenuous daily activities of an administrative nature.

  1. Stand less than 33% of time on duty.
  2. Walk less than 33% of the time on duty.
  3. Sit between 33% and 66% of time on duty.
  4. Use of hands to finger, handle or feel over 66% of time on duty.
  5. Reach with hands or arms less than 33% of time.
  6. Full ability to talk and hear.
  7. Correctable vision required.
  8. Able to lift/carry up to 15 lbs.

Minimum Educational and General Qualifications: At minimum, Associate Degree or equivalent and one year experience in healthcare or elderly care, admissions, intake, enrollment or similar services. Must be able to work effectively in a team environment and possess excellent communication, interpersonal and conflict resolution skills. Able to work with adult geriatric patients and their caregivers in a team and home environment. Required to pass the LA state exam for MDS-HC certification within one month of date of hire or the closest time to LA scheduled examinations. Employee must maintain certification prior to expiration.
