Services include but are not limited to community linkage, skills building, problem solving, and social support. Services may be delivered in the office, in clients’ homes, or in community‑based settings. <br><br> </div><div> <strong><br>Essential Duties and Responsibilities</strong> <br><br> </div><ul> <li>Provide evidence‑based mental health counseling services to individuals, couples, and/or families </li> <li>Provide therapeutic services to children, adolescents, and teens, as well as adults and older adults, in accordance with best practices and developmental needs </li> <li>Conduct comprehensive clinical assessments </li> <li>Prepare, implement, and evaluate the effectiveness of individualized treatment plans </li> <li>Conduct risk assessments and provide crisis intervention services as needed </li> <li>Utilize an electronic health records (EHR) platform for clinical documentation and care coordination </li> <li>Complete clinical documentation accurately and in a timely manner, in accordance with organizational and regulatory requirements </li> <li>Connect clients to appropriate community resources and provide ongoing skills‑building, problem‑solving, and social support </li> <li>Collaborate effectively as a member of a multidisciplinary and diverse care team </li> <li>Demonstrate strong communication, problem‑solving, and interpersonal skills </li> <li>Maintain the flexibility and professionalism required to work with a diverse client population </li> <li>Learn, follow, and uphold Carex standards, policies, and best practices </li> </ul><div> </div><div> <strong><br>Qualifications</strong> <br><br> </div><ul> <li>Current Licensed Social Worker (LSW/LPC) licensure in the state of Ohio </li> <li>Minimum of a bachelor's degree in social work or a related field with 1-2 years of experience </li> <li>Experience working with children and adolescents in a behavioral health or social services setting </li> <li>Experience providing evidence‑based mental health interventions </li> <li>Experience using electronic health records (EHR) systems preferred </li> </ul><div> </div><div> <strong><br>Working Conditions & Physical Requirements</strong> <br><br> </div><div> <br>This position involves a combination of office‑based work and community fieldwork, including services provided in client homes, schools, hospitals, and other community locations. <br><br> </div><ul> <li>Standard work hours are Monday through Friday, with flexibility required for occasional evening or weekend appointments based on client needs </li> <li>Local travel of approximately 10–15% is required; reliable transportation and a valid driver’s license are necessary </li> <li>The role may involve working in high‑stress environments and interacting with individuals experiencing crisis, trauma, or behavioral challenges </li> <li>Employees must follow all agency safety protocols during field visits, including situational awareness and de‑escalation techniques </li> </ul><div> </div><div> <strong><br>Physical Requirements</strong> <br><br> </div><ul> <li>Ability to remain in a stationary position for extended periods while completing documentation or attending meetings </li> <li>Ability to move through various environments, including navigating stairs, uneven surfaces, and confined spaces during field visits </li> <li>Ability to operate standard office equipment (e.g., computer, phone, copier) </li> <li>Ability to communicate effectively with clients, families, and interdisciplinary team members in person, by phone, and in writing </li> <li>Ability to occasionally lift or carry items up to 25 pounds (e.g., case files or outreach materials) </li> <li>Sufficient visual acuity to read and interpret documents, charts, and electronic records </li> <li>Ability to maintain composure, professionalism, and sound judgment in unpredictable or emotionally intense situations </li> </ul><div> <br> </div><div> <strong>EEO Statement </strong> <br><br> </div><div>Carex is an Equal Opportunity Employer.
