Care Initiatives Hospice is currently hiring a PRN Visit Nurse.
What it takes to be a RN Hospice Visit Nurse
- Collaborate with the physician to oversee and implement personalized care plans, ensuring the active involvement of interdisciplinary teams to achieve the optimal outcomes for each patient
- Provide consistent, high-quality patient care to ensure an exceptional and seamless patient experience
- Perform initial and comprehensive assessments on all patients in accordance with policies and procedures
- Provides high-quality patient care while ensuring clear, concise, and accurate clinical documentation
- Assesses the need for plan of care updates while keeping the patient and family involved in care planning decisions

What it takes to become a Hospice Visit Nurse:
- RN with a minimum of two years of recent clinical nursing experience.
- Current, valid CPR certification.
- Demonstrated patience and a supportive and sensitive approach to providing patient, family, and caregiver support.
- Willingness and ability to provide care in facility and patient home settings.
