About this role

 

 The Homecare Physical Therapist will provide physical therapy services to clients in their home.

 

Work Schedule:  40 hrs. weekly / 1.0 FTE; Monday-Friday 8am-4:30pm, occasional weekends.

 

Required Qualifications:

  • Graduate of accredited physical therapy program 
  • Continuing education as required by state licensing boards 
  • MN or WI Physical Therapy (PT) License upon start date 
  • Must provide proof of MN and WI PT license within 4 months of start date, if both MN and WI PT licenses are not present on start date 
  • Must have a vehicle with auto insurance and a valid driver’s license and meet all criteria required under Lakeview’s Automobile Liability Guidelines 
  • Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider) 

 

Preferred Qualifications:

  • Proof of application for alternate state within 4 weeks of start, if both MN and WI PT licenses are not present on start date
