The listed pay range or pay rate reflects compensation forfull-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

Position Overview

  • Performs nursing care/treatment modalities appropriate for individual patients. Performs outpatient electronic medical record intake including (but not limited to) chief complaint, physical assessment, and vitals. Assists LIP with procedures. References policies and procedures to assure Capital Health best practices are followed.
  • Uses considerable judgment in triaging patients in crisis. Follows up with patients via phone for recent ER/Hospital admissions. Uses screening tools to select and track performance of identified patients, metrics, and clinical outcomes.
  • Supports principles from Capital Health core mission (quality care, compassion, dignity and comprehensive continuum of care). Seeks interpretation or understanding of the illness, pain, fear, anxiety or other stressful emotion. Includes social work services that address the patient, their family, and bio-psycho-social needs and behaviors. Integrates cultural, clinical, ethnic, age and other factors into the patients plan of care.
  • Provides emotional and informational support to patients and families according to established protocols. Teaches patients and families following standardized instructional plans and documenting outcomes. Acknowledges and includes patient's family during communication with the patient, as appropriate and as designated by ambulatory patient.
  • Assesses need for patient education. Plans, implements and documents patient education and evaluates and documents patient comprehension and compliance. Calls patients to explain laboratory and test results under the direction of the LIP. Explains preparations and procedures to patient.
  • Obtains and records patient history within electronic medical record. Reviews laboratory and other test results daily and prioritizes them for LIPs according to established protocol. Prepares patients, exam rooms and medical records for scheduled and non-scheduled patients.
  • Obtains specimens, identifies patients and specimens according to CH policy. Recognizes important changes in patient condition and reacts appropriately in the Ambulatory setting according to CH policy.
  • Administers and documents medications accurately and safely according to established protocol. Performs and demonstrates technical skills specific to assigned Ambulatory setting. Administers injections as ordered by LIP according to established protocol and documents.
  • E-prescribes medications to pharmacies under direction of the LIP. Performs Medication reconciliation.
  • Completes competency checklist with their preceptors.
  • Performs Quality Assurance activities as defined by Ambulatory setting. Follows ANA Code for Nurses and Capital Health System Standards of Practice.
  • Documents refrigerator temperatures for proper storage of medications and specimens and takes appropriate action if outside acceptable range.
  • May participate in Research studies depending on practice.
  • Triages and documents patient medical phone calls, assessing patients' mental status, and schedules according to established protocol.
  • Ability to assess substance abuse/misuse and withdrawal symptoms; and ability to screen patient for danger to self or others.
  • Attends required training and collaboration sessions as scheduled. An active team member.
  • Plans, reviews, establishes, and implements procedures to ensure proper communication between physician, LSCW, community resources, and patient. Helps facilitate transition between resources and identifies financial or social barriers. Refers patient to abuse/neglect/domestic violence, alcohol and substance abuse resources as needed.
  • Performs other duties as assigned. Demonstrates willingness to assist co-workers during periods of heightened patient care activity.

MINIMUM REQUIREMENTS

  • Associate's degree or graduate of accredited school of nursing. NJ or PA license based on primary State office location required. Must attain second out of state RN license (NJ or PA) within three months from date of hire.
  • Associate's degree requires eight years nursing experience in related specialty, inpatient and/or outpatient setting. Bachelor's degree requires four years nursing experience in behavioral health, or inpatient and/or outpatient setting.
  • AHA BLS - Healthcare Provider, Registered Nurse - NJ, Registered Nurse - PA
