Position Overview

ESSENTIAL FUNCTIONS

  • Supervise day center support staff including CHHAs, housekeeping, dietary, and front desk teams. Ensure staffing coverage and assist with scheduling coordination. Coordinates onboarding, training, competency review, and performance management for assigned staff.
  • Oversees scheduling and coordination of Certified Home Health Aides (CHHAs) and other support staff to meet participant needs in the home, community and center. Maintains oversight for Certified Home Health Aides (CHHA) schedules, ensuring accuracy, timeliness, and adherence to care plans.
  • Supports mealtimes, transportation flow, and center-based needs in real time.
  • Maintains a safe, clean, and welcoming environment for all participants. Ensures regulatory compliance for cleanliness, infection control, and environmental safety.
  • Addresses and documents participant concerns, incidents, and grievances promptly and appropriately. Supports quality assurance, grievance resolution, and participant satisfaction efforts.
  • Leads daily huddles and regular staff meetings to ensure effective team communication and accountability. Promotes teamwork and open communication across disciplines to enhance participant outcomes.
  • Ensures adherence to PACE regulatory standards, HIPAA, and infection control procedures.
  • Maintains logs, complete incident reports, and track daily attendance.
  • Prepares for audits, surveys, and assist with recordkeeping as needed.
  • Coordinates facility maintenance and inspections with Capital Health teams and vendors.
  • Serves as a liaison among the Center Director, IDT, and external partners.
  • Supports program development and operational improvements aligned with Capital Health and PACE goals
  • Performs other duties as assigned.

MINIMUM REQUIREMENTS

  • Bachelor’s degree in healthcare administration, gerontology, social services, public policy, or related field. Equivalent combination of education and relevant experience in healthcare, senior services, or PACE operations in lieu of degree.
