To further this mission, Callen-Lorde promotes health education and wellness, and advocates for LGBTQ+ health issues.    About the Role: As a Supervisor of Care Coordination, you will play a critical role in overseeing care coordination services that reduce barriers to care and improve patient health outcomes. This role provides day-to-day leadership, guidance, and oversight to care coordination staff while ensuring patients receive the tools, resources, and support needed to successfully navigate healthcare and social service systems. The Supervisor serves as a key link between clinical teams, social services, and community partners to support high-quality, patient-centered care.   Key Responsibilities:   As a Supervisor of Care Coordination, your responsibilities will include: Team Leadership: Supervise a team of Patient Navigators, Case Managers, and Supportive Case Managers, providing guidance, support, and performance evaluations. Case Management: Provide direct case management services, including intake, assessment, treatment planning, and ongoing reassessment. Care Coordination: Coordinate care with medical providers, behavioral health professionals, and other community resources to ensure seamless patient care. Entitlements and Referrals: Assist patients in accessing entitlements and referring them to appropriate services. Documentation: Maintain accurate and timely documentation of all patient encounters and activities. Performance Management: Monitor staff performance, provide feedback, and address any performance issues. Quality Improvement: Participate in quality improvement initiatives and contribute to the development of best practices. Professional Development: Attend conferences, trainings, and workshops to stay updated on the latest trends and best practices in care coordination.   Work Schedule:  This is a full-time position with evening hours.    Benefits:  Salary will be based on experience and will be accompanied by a comprehensive benefits package including an exceptional low cost medical plan option for you, your spouse/domestic partner and/or your family, dental insurance, vision insurance, no cost life insurance, short- and long-term disability insurance, Flexible Spending Account, Tuition Assistance, commuter benefits, a generous paid time off plan, and a 403B retirement savings plan.  QualificationsEducation, Experience Licensed Master of Social Work degree (LMSW), Licensed Mental Health Counselor (LMHC) or related Master’s degree preferred or a bachelor’s degree with a minimum of 3 years of experience in providing direct case management services will be considered. Experience supervising case management staff and performance based grant contract monitoring strongly preferred.   Desired Characteristics, Skills Ability to work within team-based structure.
