Conducts physical wound rounds routinely at each location based on current trends and needs. Collects data and information relative to the health or situation of the resident with wound, ostomy, and/or continence care needs. Reviews existing orders and makes recommendations to physicians, managers, and other healthcare team members focused on healing, maintaining, or supporting the residents' wound care goals. Conducts Medical Record review of existing orders and makes recommendations to physicians, managers, and other members of the healthcare team that are focused on healing, maintaining, or supporting wound care goals. Monitors for compliance of the facility's wound care systems. Analyzes assessment data to determine actual or potential problems/issues related to wounds, ostomy, and/or continence care needs. Assists facility nursing leadership in developing and implementing plans for wounds, ostomy, and/or continence care that are focused on strategies to optimize the facility's wound systems. Assists facilities in developing, providing, and evaluating staff education programs, orientation, continuing education, and resident/family education as it relates to wound, ostomy, and incontinence care as appropriate. Participates in and attends staff development department meetings.
