Summary
Eligibility determination: Conduct the pre-admission face-to-face visit required for certifying a patient's terminal illness and appropriateness for hospice care. This role will be responsible for conducting face to face visits on our recertifications. Initial assessment: Perform comprehensive physical, emotional, spiritual, and psychosocial assessments of the patient and family in their place of residence, which can be a private home, hospital, or long-term care facility. Address concerns: Listen to and address the concerns of the patient and family, helping them feel comfortable and supported during the transition to hospice. Symptom management: Evaluate and manage a patient's pain and other distressing symptoms such as nausea, shortness of breath, fatigue and other distressing symptoms. Care planning: Work with the interdisciplinary team and the patient's attending physician to develop, implement, and document a personalized care plan to assure the patients’ goals and wishes are prioritized. Coordinate services: Assist with the coordination of necessary services, including medications, treatments, and durable medical equipment. Discuss end-of-life wishes: Facilitate serious illness conversations with patients and families to assist with advanced care planning and end-of-life decisions. Collaborate with the team: Consult with the hospice medical director, attending physician, and interdisciplinary team members (including social workers and chaplains) to ensure continuity and quality of care. Complete paperwork: maintain accurate and timely patient records. documenting assessments, interventions and communications in the electronic health record (EHR). May be assigned other duties as appropriate.



Job Qualifications



  • Master's or doctoral degree: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP from an accredited program is required

  • Current, active, and unrestricted Missouri Nurse Practitioner license in good standing is mandatory

  • Minimum one year as practicing Nurse Practitioner

  • Strong organizational skills

  • Ability to function independently while making sound nursing judgements

  • Must have excellent one-on-one and telephone communication skills

  • Computer skills

  • Ability to stand and walk for long periods

  • Ability to provide two continuous hours of standing and seated exercise with and without weights

  • Ability to travel to various Bethesda facilities and Community locations to provide services

  • Ability to transfer and move patients utilizing proper body mechanics

  • Ability to lift heavy objects using proper lifting techniques and with assistance as necessary

  • Manual dexterity, coordination and skillful use of hands when working with clients, staff and equipment

  • Ability to utilize computer and physically able to perform necessary written documentation


 


Pay range: $48.88 - $70.88 per hour
