Benchmark Human Services has grown to become one of the most respected leaders in the country in the areas of intellectual and developmental disabilities (IDD) and behavioral health. We are launching a new program in Vermont designed to support individuals and families in planning, developing, selecting, accessing, coordinating, and overseeing medical, social, educational, and other services and supports. This includes providing planning, advocacy, monitoring, and guidance to help them make and evaluate their own decisions. We are hiring a Regional Case Manager Supervisor to provide case management services and administrative oversight for the company’s short- and long-term goals within designated regions by ensuring that staffing, financial, compliance, customer service and more importantly the goals and needs of those served are met.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Follow all Vermont state statutes, rules, definitions, program requirements, program specific services, variances and exceptions
  • Provide case management duties with designated cases and when staffing levels or consumer needs are not being met.
  • Hires, trains, manages, develops and coaches staff to maximize performance and outcomes
  • Establishes staffing levels and monitors caseloads to maximize performance and financial resources
  • Establishes and maintains positive relationships with individual, families, guardians, state officials and team members
  • Establish and monitor quality outcomes for consumers and the region
  • Ensures that all case records, plans and other related documents are properly maintained
  • Coordinate meetings with individual, families and guardians, and members of the interdisciplinary team as required
  • Records work and billing time in accordance with company policies
  • Tracks and monitors specified data points
  • Take necessary action to maximize revenue and limit expenses
  • Participate in short and long term planning process to ensure continued quality and financial success
  • Report any suspected abuse, neglect or exploitation immediately to supervisor or department head.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Must have knowledge of Vermont’s public service system for developmental disabilities services and brain injury services
  • Fundamental understanding of state statutes, rules, definitions, program requirements, program specific services, variances and exceptions.
  • Must be able to work collaboratively while meeting individual performance goals
  • Must be able to verbally and non – verbally communicate with a broad audience of stakeholders
  • Ability and skill to use technology tools in the daily performance of duties
  • Effectively lead a team of case managers toward meeting customer service, compliance and financial outcomes
  • Analytical and critical thinking skills
  • Ability to manage financial resources

QUALIFICATIONS:

  • 2 years of case management experience provided in Vermont to people with developmental disabilities and/or brain injury and meet QDDP qualifications for at least one year.
  • Qualified Developmental Disability Professional as defined by the Agency of Human Service which is a person who has at least 1 year of experience working directly with people with intellectual or development disability and is one of the following: RN, Doctor of Medicine or Osteopathy, a licensed and certified professional with a Bachelor's Degree, OT, PT, PTA, Social Worker, Psychologist, SLP, Dietician, Recreational Professional or QDDP.
  • Preferred credentials include Accredited Case Manager (ACM) or Certified Case Manager (CCM)
  • Valid Driver's License
  • Valid Auto Insurance
  • At least 2 years working in a case management role in Vermont
  • At least 3 years working in a supervisory or management role
  • Ability to travel up to 75%
