JOB SUMMARY

Under the supervision of the Pharmacy Operations Manager and/or Pharmacy Operations Supervisor, performs all duties necessary to provide for the efficient daily operation of the pharmacy, providing quality products and clinical services to home and community care patients.

 

DUTIES AND RESPONSIBILITIES

·       Performs and documents all activities of dispensing medication including but not limited to: clinical review, pre-check, visual verification of sterile compounding, and final check of prepared product according to Bayshore Policies and Procedures (P&P) and the model standards for pharmacy compounding outlined by the National Association of Pharmacy Regulatory Authorities (NAPRA) and adopted by the Ontario College of Pharmacists (OCP):

o   Reviews medical order for clinical appropriateness (including but not limited to individual patient bloodwork, medication list, diagnosis, and best practice guidelines) for each individual patient.

o   Prepares and/or verifies infusion supply order for each individual patient to ensure accuracy related to patient-specific medical order.

o   Prepares and/or verifies compounding instructions for accuracy related to ingredient calculations and infusion pump programming requirements.

o   Prepares and/or verifies infusion equipment programming to ensure accuracy and patient- and/or medication-specific therapeutic administration requirements are met.

o   Oversees sterile compounding via visual verification of each step prior to release to final check.

o   Checks final sterile compounded product for particulates and to ensure labeling meets standards required for each individual patient prescription prior to dispensing.

·       Obtains, interprets, and evaluates all medical orders to ensure therapeutic appropriateness, cost effectiveness, accuracy, and completeness.

·       Confirms accuracy, completeness, and final check of compounded sterile product and noncompound medications prepared for release, and pump programming if applicable, in accordance with established procedures and applicable laws.

·       Ensures appropriate supplies ordered based on patient specific medical order and accurate data entry prior to release of prescription.

·       Ensures compliance with federal, provincial, and local laws and regulations, Bayshore Policies and Procedures (P&P) and accrediting body standards (Ontario College of Pharmacists (OCP), Bureau Veritas (ISO)).

·       Adheres to sterility, stability, and compatibility guidelines when establishing Beyond Use Date for sterile compounded products as outlined by approved drug references and National Association of Pharmacy Regulatory Authorities (NAPRA) and Ontario College of Pharmacists standards.

·       Adheres to quality control procedures as indicated in Bayshore Policies to meet ISO standards.

·       Performs accurate computer prescription order-entry, labeling and dispensing in accordance with Ontario College of Pharmacists (OCP) regulations.

·       Collaborates with health care providers in relation to pharmaceutical and clinical expertise as required.

·       Initiates, reviews, and updates all required documentation per Bayshore Policy and Procedure (P&P) in accordance with relevant legislation.

·       Accurately maintains all legally required records as outlined by Bayshore Policy and Procedure (P&P) and accreditation standards (Ontario College of Pharmacists (OCP), Bureau Veritas (ISO)).

·       Provides clinical and technical information and communicates results of monitoring activities to patients and care teams (for example: caregivers, nurses, prescribers, etc.) regarding dispensed medication, equipment, and supplies including proper utilization, necessary therapeutic actions, and possible adverse reactions, and/or drug interactions, as required.

·       Documents all individual patient-related communications under the specific patient profile in the prescription processing software.

·       Notifies appropriate health care team if there are changes in the therapy for individual patients.

·       Follows established procedures for obtaining medication, equipment, and supplies ensuring adequate inventory levels and preferred product selection of medications, infusion solutions, equipment, and supplies.

·       Provides individual patient counselling for all new prescriptions and as requested by the patient or caregiver, including updating current medication list.

·       Regularly reviews prescription refills for ongoing treatment requirement for each individual patient.

·       Works cohesively with, and delegates appropriately to, other Bayshore team members including Customer Service Representatives, Pharmacy Assistants, Registered Pharmacy Technicians, Registered Pharmacists, Warehouse Technicians, Logistics Coordinators, Medical Device Reprocessing Technicians, Medical Device Management Coordinators, Nurse Clinical Consultants, and Contracts Coordinators.

·       Monitors selected measures of individual performance as it relates to departmental performance standards and reports results as requested.

·       Participates in Performance Improvement activities and continuous professional development as required.

·       Attends and participates in departmental and business meetings as scheduled.

·       Assists with tracking narcotics and controlled substances as required by the Ontario College of Pharmacists (OCP) and Health Canada (HC) through the established narcotic reconciliation procedure.

·       Provides virtual or in-person medication reconciliation for individual patients as requested.

·       Assists in orientation and training of new employees.

·       Performs other duties as assigned.



QUALIFICATIONS

Education and/or Experience

·       Bachelor of Science (BSc) degree in Pharmacy or a PharmD, from an accredited college or university.

·       Experience in a sterile compounding or equivalent work experience is an asset.

 

Language Skills

·       Ability to read, analyze, interpret professional journals, technical procedures, and government regulations.

·       Ability to write clinical correspondence to members of individual patient care team and documentation for patient chart.

·       Ability to effectively present information and respond to questions from managers/supervisors, patients/caregivers, nurses, prescribers, and other members of the patient care team.

 

Mathematical Skills

·       Must be able to perform accurate pharmaceutical calculations as related to sterile compounding.

·       Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals (including conversion between mEq and mmol for various electrolytes).

·       Ability to compute rate, ratio, and percent, and to interpret graphs.

·       Ability to solve word problems and algebraic equations.

 

Reasoning Ability

·       Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.

·       Ability to define problems, collect data, establish facts and draw valid conclusions.

·       Interpret instructions provided in written, oral, or visual form

 

Certificates, Licenses, Registrations

·       Licensed with Ontario College of Pharmacists (OCP) in good standing under Part A of the register, to be able to provide patient care and participate in patient care activities.

·       Current CPR certification.

·       Sterile compounding certificate is an asset.

 

Other Skills and Abilities

·       Proficient with basic computer programs (Microsoft Office).

·       Experience with Kroll is an asset.

·       Sterile compounding skills are an asset
