About the facility 

Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.

 

This role is based at one of those sites, the Corio Community Health Centre, an integral part of the community that has been providing health care since 1976. The Centre, and the programs that are located at the centre are a much loved and respected part of the community.

 

About the role

 

To contribute to and be part of the community - based, multidisciplinary area health team to support clients and carers to achieve an optimum level of health independence through:

 

  • Comprehensive, co-ordinated and integrated multidisciplinary services
  • To assist in the provision of client –centred care to community members from a broad range of backgrounds
  • Contribute to the development and delivery of high quality, cost effective community-based services
  • Contribute to meeting the program and area health team key performance indicators (KPI’s)
