Job DetailsJob Location: Avow Hospice - Naples, FL 34105Salary Range: $87,360.00 - $98,000.00 Salary/yearJob Summary:

Provides clinical and fiscal management of team ensuring quality of care and customer satisfaction. Directly supervises all assigned staff representing all clinical disciplines and coordinates competency assessments with established discipline specific resources. Serves as a lead resource for their licensed discipline.

Responsibilities:

Instruct staff on their responsibility in following the Compliance and Ethics Program.
Detect non-compliance and follow up as appropriate.
Acts as a professional and role model.
Provides clinical resource, problem solving and skill development to team members.
Participates in General and Clinical Orientation for new staff.
Facilitates the education of services focused on patient, family, caregiver, bereaved, and facility staff needs in collaboration with the clinical leader.

Provides clinical coordination by:

Facilitating team meetings including Interdisciplinary Group- IDG.
Review report from previous shift(s) and communicate information to the team members as appropriate.
Monitoring staff caseloads and productivity expectations
Ensuring time-sensitive deadlines (i.e. re-certifications, follow-up/comprehensive visit expectations, etc.) are met.

Performs clinical evaluation of patient care by:

Routinely auditing clinical documentation for completeness and appropriateness of documentation, indications of POC driven service, timeliness of documentation, interdisciplinary communication, continuity of care, and compliance with policies, best practices, and regulations.
Monitoring Discipline-specific Model Visit expectations for weekly productivity; redesigns model visit, when appropriate, to meet changing needs.
Meeting regularly with assigned team to review staff goals, issues and concerns
Participating actively in planning and carrying out QAPI efforts.
Reviewing monthly and quarterly QAPI reports for knowledge/performance deficits and education opportunities in collaboration with their team leader.
Collaborates with peers and with those in all levels within the organization to build a sense of team.
Ensures compliance to all accreditation and regulatory requirements under their supervision.
Represents the organization through public speaking opportunities or facility educational workshops.
Completes, or as appropriate oversees, the timely completion of staff evaluations, competencies, supervisory visits, and mandatory education expectations.
Informs their direct supervisor of any performance, training or quality of care issues and solutions within the team.
Assists with complex cases to monitor and improve quality of care; does case review with staff members as needed in collaboration with their team leader.
Identifies, monitors and provides training as needed regarding new practices, policies and procedures to ensure compliance with new or changed components of clinical care.
Participates on committees and task-forces as assigned.

Specialty certification in Hospice or other specialties as applicable within 2 years of obtaining the position Valid Driver’s license from the State of Florida Registered Nurse License in the State of Florida or Licensed Clinical Social Worker (LCSW) within 90 days of role Cardiopulmonary Resuscitation (CPR) certification required (BLS from approved provider) if a Registered Nurse

Education And Experience:
Master's degree from four-year accredited college or university with hospice/home care and management experience preferred. Bachelor’s degree from a four-year college with 2-3 years hospice/home care and management experience or equivalent education/experience combination considered. Required to start a Bachelor’s program within 6 months of hire into position and complete within 3 years.

Computer/Communication Skills:
Communication skills of reading, writing, and speaking in English. Knowledge of word, excel, and power point is preferred.

Supervisory Responsibilities:
Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Language Skills:
Communication skills of reading, writing, and speaking in English, bi-lingual Spanish preferred.

Mathematical Skills:
Math skills including addition, subtraction, multiplication, division, and percentages/fractions. Required to pass math test at hire and at skill fair yearly for Registered Nurse.

Other Skills And Abilities:
Ability to teach and present information. Ability to coordinate multiple tasks, patient needs and prioritize and assign appropriate staff resources to fulfill those needs

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
