A Brief Overview

The RN coordinator creates a single point of contact for transferring patients to the most appropriate Avera hospital within our system. This will include collaboration regarding each patient's unique needs and identifying the most appropriate facility. This position supports the assignment, tracking and problem solving so that patient placement can occur in a timely manner. The RN works collaboratively with the team members, nursing departments, outlying facilities, as well as the physicians to facilitate efficient movement of patients throughout the footprint. The RN will serve as a link in gathering clinical data to ensure continuity of patient care and appropriate patient placement. The RN will monitor all transfers, identifying potential delays in care and provide communication between members of the care team.

What you will do

  • Oversees the flow of patients from outlying facilities and clinics to ensure that the goals of an efficient and seamless entry into the facility occurs.
  • Prioritizes and places patients in the appropriate regional sites by using critical thinking skills and following department policies and guidelines.
  • Works as a liaison with bed placement coordinator to ensure timely transfer of patients.
  • Evaluates the transfer center process toward attainment of the outcomes.
  • Collaborates with physicians and nurses as well as health care providers, including specialists in assessing patient placement.
  • Utilizes research/evidence-based findings in practice if appropriate.
  • Recognizes the need for knowledge and skills sufficient to integrate the use of healthcare technology and/or information management systems.

Required Education, License/Certification, or Work Experience:

  • Registered Nurse (RN) - Board of Nursing An active license in state of practice Upon Hire

Preferred Education, License/Certification, or Work Experience:

  • 1-3 years Nursing Experience
