Overview: Coordinates designated activities and functions of the Centerwide Safety and Performance Improvement, Utilization Review, Disclosure Management, Clinical Trial and related Administrative functions and processes in accordance with Agency Policies & Procedures.   ESSENTIAL/CORE JOB-SPECIFIC DUTIES AND RESPONSIBILITIES   1.         Coordinates the departmental functions of the UR, PI, Clinical Trials, and Disclosure Management departments including supervision of assigned staff.  Oversees scheduling, agenda development,             Coordination, meeting documentation and follow-up for the Centerwide Performance Improvement Committee, Patient Safety Committee and UR Subcommittee.  Coordinates agenda development for the quarterly Clinical Staff Meetings and  attends Monthly Peer Review Meetings.   2.         Coordinates Center-wide safety processes in consultation with the Safety Director in accordance with policies and procedures, including the Safety Suggestion Program.  Responds to concerns/e-mails received through the Apalachee website as well as calls and letters.  Follow-up as indicated on all safety issues. Serves as Patient Safety Coordinator.  Oversees annual Failure modes and Effects Analysis (FMEA) including selecting workgroup members, documenting process, recommendations, and follow-up.  Completes formal annual review and update of safety policies and plans (170 series) in conjunction with key staff to ensure compliance with applicable regulations and standards (e.g. Joint Commission, OSHA, CDC, AHCA licensure, etc.).   3.         Serves as the Agency’s designated Risk Manager for hospital licensure purposes. Coordinates Agency Risk Management Program pursuant to applicable sections of Florida Statutes and Florida Administrative Code. Completes and submits annual report of incidents to the Governing Board and Agency for Health Care Administration. Provides for or arranges for orientation and annual in-service training to staff regarding incident reporting and risk management. Completes quarterly, written PI / Risk Management Report for submission to the Board of Directors.  Preserves confidentiality and security of all information related to incidents, complaints, claims, investigations, and other risk management responsibilities.  Coordinates Root Cause Analysis (RCA) reviews for identified incidents.  Completes reports, monitors implementation of identified improvements.   4.         Coordinates internal and external site visits and offsite record requests, including development of any needed corrective action plans, monitoring and follow-up (i.e., Joint Commission, Program Review, ME, AHCA, etc.).  Oversees designated external Agency reporting requirements in accordance with C&F contracts and standards.  Coordinates data collection and reporting of HBIPS data.  Coordinates completion of assigned sections of the Joint Commission Periodic Performance Review Process.  Monitors corrective active plans for identified standards.   5.         Coordinates annual reviews and updates of Centerwide Policies and Procedures. Coordinates annual updates of the Agency’s written Strategic Plan, PI Plan, and UR Plan.   6.         Conducts Seclusion/Restraint review and monitoring including receipt of all placement forms corresponding with logs, follows up with any variances.  Reviews placement forms for completion of all essential items and congruence with supervisory review forms.  Tabulation of utilization data, preparation of trending charts, dissemination of reports to program directors and Patient Safety Committee. QualificationsEducation/Experience:  Master's degree with a major in psychology, social work, counseling, or a related human services field or Bachelors degree and 5 years of related experience. Two years of related professional administrative experience in performance improvement/quality assurance / patient safety preferred. Professional clinical experience in a psychiatric / community mental health setting preferred   Licensure or Certification: Must possess a valid Florida driver’s license Maintain authorization as an approved Center vehicle driver.
