The Prevention Navigator is responsible for leading, implementing and integrating the Mission and Core Values of WORLD as well as AHF’s Management Philosophy in the leadership of the programs within and outside WORLD and AIDS Healthcare Foundation.

 

Summary

 

As part of the WORLD staff the Prevention Navigator will interact effectively and consistently to provide substance misuse and HIV and Hepatitis prevention to racial/ethnic minority males at risk for HIV/AIDS, including males who have sex with other males (MSM), transgender individuals, as well as those who identify as LBGTQ+ who are not in stable housing and reside in communities with high incidence rates of HIV. Services include providing intervention and educational workshops, promoting mental health and providing HIV testing and making referrals for STI and HCV testing for clients. Under the direct supervision of the Associate Executive Director, the Prevention Navigator works in collaboration with WORLD staff, community partners, and SAMHSA staff to ensure that services are delivered to the population of focus.

 

Essential Duties & Responsibilities

Includes the following.  Other duties may be assigned.

 

  • Facilitate Seeking Safety behavioral intervention and educational workshops on substance misuse and HIV and Hepatitis education via workshops and training.
  • Conduct required individual client interviews and testing for HIV with referrals to STI and Hepatitis C testing.
  • Develop recruitment strategies and work with community partners to recruit and screen eligible program participants.
  • Assist clients with navigating services to link individuals to mental health disorder, substance misuse, HIV and Hepatitis testing, care and supportive services through referrals to WORLD’s programs or other community service providers as needed.
  • Maintain client records including timely documentation (and workshop evaluations) in the electronic health record system or other database in accordance with clinical policies and procedures.
  • Develop and maintain monthly reports on recruitment and retention to be submitted to the supervisor.
  • Lead preparatory activities for implementing the intervention including the creation of participant handbooks and printing program material to be used in each session.
  • Contribute to and manage social media forums for program promotion and recruitment.
  • Provides non-directive information to clients about HIV disease, STI and HCV treatments, clinical trials, stress reduction and self-care strategies.
  • Attend ongoing training on program intervention planning and implementation.
  • Helps clients identify risk reduction strategies
  • Follow up with new/returning clients after their initial/return appointment regarding questions, concerns or barriers to care and conduct Seeking Safety intervention sessions to increase program retention.
  • Travel to various locations to host HIV/AIDS, STI, HCV, and Mental Health awareness events as needed.
  • Must be willing to work a flexible schedule including some weekends.
  • Participates in creating and maintaining a workplace that values diversity and is free of racism, sexism, heterosexism and other discriminatory practices.

Qualifications

Education

Bachelor's degree in Human Services, Psychology, Social Work or related field or equivalent educational and work experience. Seeking Safety, HIV educational and HIV testing preferred. Knowledge of harm reduction, healthy coping, and steps for networking with providers preferred.

 

Experience

At least 3 years of related experience, including two (2) years of outreach and testing experience.

 

Other Requirements

Knowledge and understanding of substance misuse, HIV/AIDS and Hepatitis prevention and mental health promotion. Excellent  group and individual communication skills with clients, medical professionals, paraprofessionals, and coworkers; Strong group facilitation skills and public speaking skills; Ability to work late some nights and weekends. Bilingual English/Spanish speaking preferred.

 

Computer/Software Skills & Abilities

To perform this job successfully, an individual should have knowledge of Microsoft Word, Excel, Outlook, PowerPoint and Adobe Acrobat Reader.
