Job Summary

The SCL (Supports for Community Living) Coordinator participates in Plan of Care (POC) meetings and assessments, with SCL/MPW team members or outside agency support staff, as appropriate. The SCL Coordinator encourages implementation of member goals with the objective of increasing self-reliance and independence and ensures that required service documentation reflects progress and concerns.  The SCL coordinator ensures that activities are designed based on the needs and interests of members and not only entertain, but they also encourage interaction, relaxation, promote health, and contribute to the overall quality of our members’ days.  The SCL Coordinator is expected to lead by example - maintaining a positive attitude; being a thoughtful, supportive, productive and dependable leader and co-worker; keeping our members safe; and always working to enhance the health and happiness of the people we serve and those who care for them. 

 

Qualifications

  1. High school diploma or GED required; Bachelor of Science Degree preferred; additional requirements as mandated by state-specific regulatory guidelines. 
  2. Minimum of two years (or full-time equivalent) activities experience preferably in a healthcare setting; additional requirements as mandated by state-specific regulatory guidelines (e.g. Certified Activity Professional).   
  3. Prior experience working with an SCL provider and/or successful completion of classes under the College of Direct Support strongly preferred.
  4. Prior experience working with seniors and/or adults with special needs preferred.
  5. Strong communication and engaging interpersonal skills. 
  6. Able to multi-task and manage time effectively
  7. Able to work a flexible schedule.
  8. Able to stand and actively engage with members for an extended period; able to bend, pull, push, squat, turn, transfer and lift 50 pounds.  
  9. Working knowledge of Microsoft Office Word, Excel and Outlook.
  10. Successful results of background check, and where applicable, additional state-specific regulatory requirements.    

 

Job Responsibilities

  1. Participate in member Plan of Care (POC) meetings and assessments, with SCL/MPW team members or outside agency support staff, as appropriate. 
  2. Collaborate effectively with member case managers and other support providers/agencies to ensure that services are person centered.
  3. Advocate for and be a resource to members and their families. Allow self-determination and input from families to guide services
  4. Encourage implementation of member goals with the objective of increasing self-reliance and independence for members, while also ensuring required service documentation reflects progress and/or concerns. 
  5. Develop, plan, organize and implement diverse and creative therapeutic recreational programs for members; draft and publish related calendars and communications.
  6. Train staff with Behavior Specialist as well as train staff on each SCL member’s individual need while ensuring all SCL goals are met for members on the SCL waiver
  7. Ensures compliance with the federal Preadmission Screening and Resident Review process and also ensures all paperwork associated with members covered by the SCL waiver is complete and on time, including but not limited to:  Monthly SCL notes, Annual Paperwork, daily in and out book and monthly vouchers for transportation.
  8. Actively participate in staff meetings and in-service programs; collaborate with cross-functional team, providing feedback and suggestions for enhancing the member experience and achieving the center’s overall goals & objectives. 
  9. Stay current on related industry knowledge and trends through educational opportunities, publications, networking, and participation in professional organizations.
  10. Assist members with activities of daily living as needed. 
  11. Communicate in a positive, courteous and professional manner with members and their families, co-workers, referral agencies, and the community at large.
  12. Comply with company policies and procedures, safety and regulatory laws and standards.
  13. Other duties and responsibilities as changed or assigned at any time.

Our experienced adult medical day care staff has the ability to monitor your health and help with daily activities, including personal care and administration of medications. Our clients enjoy our delicious and nutritious meals daily, as well as activities, trips, and new friendships.

Our onsite nurse and social worker will make sure all of your needs are attended to well after you have left our facility.

The facility operates in two sessions, from 8:00am to 1:00pm, and from 1:30pm to 6:30pm, Monday through Friday, allowing program participants and caregivers flexible options while planning attendance and care.
