Location: Mt. Vernon, NY  

Hours: M-F 9 am to 5 pm  

Rate of Pay: $28.50 to $35 depending on certification

CASAC-T required at a minimum 

 

Maintain a caseload of 50+ patients and perform casework responsibilities:  

 

  • Perform intake interviews with patients to evaluate appropriateness for treatment, compile all information required to process the patient's request for service.
  • Write complete and accurate Intake Bio-Psycho-Social assessment within required timeframe. Include the patient’s history in all areas, as well as an assessment of the influences and effects of dependencies.
  • Establish collaborative relationships with patients within a supportive atmosphere to assist them in confronting drug related behavior and achieving sobriety.
  • Know and use age-appropriate interventions with patients.
  • Meets with patient as noted in treatment plan.
  • Provide individual, group and family counseling.
  • Strives to educate patients in all areas of substance abuse and recovery.
  • Complete clinical / counseling records as required in accordance with The Guidance Center’s policies and procedures and in compliance with all relevant rules and regulations  
  • Develop multi-disciplinary treatment plans address individualized patient needs with realistic goals & objectives within required timeframe.
  • Progress notes reflect problems, goals, objectives and interventions as described in treatment plan, notes are accurate and completed within the time period allowed.  
  • Complete Discharge Summaries within the time period allowed.  

  

Demonstrate thorough knowledge of State & Federal laws as well as clinic policies and procedures:  

  

  • Take an active role in keeping abreast of changes & updates in agency and CDTC policies and procedures.  
  • Interpret clinic policies and regulations to patients and assist patients in adhering to them.  
  • Maintain patient confidentiality in accordance with CFR Part 42.  

  

 

Coordinates patient care with all members of the treatment team:  

 

  • Represent patients and participate as a team member in Interdisciplinary Team meetings.  
  • Act as liaison to other community agencies and governmental organizations.  
  • Makes appropriate referrals to outside agencies in accordance with patient needs.  
  • Attend weekly group and individual supervision.  
  • Takes on other responsibilities as requested.  
  • Respects and supports decisions made by interdisciplinary team in regard to patient care.  
  • Act responsibly in following through on team decisions.  
  • Maintains a positive attitude in situations requiring conflict resolution.  
  • Demonstrates an understanding of specific issues/concerns for varying age and/or gender population served.  
  • Assess crisis situations in regard to patient care and reports pertinent observations to appropriate staff.  
  • Able to assess evidence of child abuse/neglect and/or domestic violence with prompt reporting to supervisor.
