Location: Monticello, NY

Pay Rate: $22.40 per hour

$2,500 Sign-On Bonus payable according to agency policy. 

Hours: Monday-Friday 8:30AM-5:00PM (40hrs.)

 

OVERVIEW OF PRIMARY RESPONSIBILITIES:

The Supported Housing Program provides housing and supportive services to homeless adults, chronically homeless adults, and adults at risk of homelessness. The program also serves individuals with Serious Mental Illness (SMI), adults aging out of foster care (AOFC), and Veterans.

 

PRIMARY FUNCTIONS:

Key Responsibilities

  • Work with individuals presenting complex needs, including crisis intervention and stabilization.
  • Engage participants in developing a collaborative Housing, Health, and Recovery Plan focused on self-sufficiency.
  • Provide comprehensive services addressing all life domains, including employment, finances, medical care, education, transportation, relationships, safety, culture, spirituality, and social supports.
  • Support participants in integrating into the community of their choice and making informed decisions about services, supports, and recreational opportunities.
  • Assist participants in planning for discharge from Supported Housing, including identifying and coordinating appropriate supports to ensure a successful transition.
  • Serve as a role model and support participants in achieving the highest possible level of independent community living.
  • Maintain compliance with all required documentation, agency policies, and NYS Office of Mental Health (OMH) regulations.
  • Collaborate closely with community providers to facilitate smooth transitions and continuity of care.

QUALIFICATIONS AND ATTRIBUTES:

The ideal candidate will have experience working with high-need populations and the ability to respond effectively in crisis situations. This role supports participants in achieving housing stability, recovery, and increased independence through person-centered, strength-based services

  • Minimum of two (2) years of experience in case management, mental health services, or equivalent experience
  • Excellent interpersonal and engagement skills
  • Strong organizational abilities

EDUCATION AND EXPERIENCE:

  • Bachelor’s Degree in Health and Human Services or a related field, or equivalent experience
