Summary

Collaborates with members of the healthcare team to provide Medical Nutrition Therapy for Home Infusion and Acute Care Hospital patients of all ages. Facilitates discharge planning and coordinating nutrition care for nutrition support patients including triage for inpatient/outpatients. Time will be split based on census and staffing needs (approximately 50/50).

 

Position Reports To: Nutrition Therapy & Diabetes Manager


Essential Functions

  1. Provides nutritional care to patients in assigned area in a timely fashion per policy.
  2. Work with interdisciplinary team members to provide quality nutrition care to patients.
  3. Assess nutrition needs and identify nutrition requirements specific to disease state and patient age.
  4. Provide nutrition prescriptions, recommendations and education for pediatric and adult Home Infusion patients including oral nutrition supplements, enteral formulas and TPN.
  5. Provide pump education and methods of nutrition administration for inpatients and outpatients.
  6. Work closely with families, care givers and health professionals to provide optimal nutrition for high risk patients.
  7. May include home visits or Telehealth.
  8. Coordinates and facilitates the Nutrition Support Care Plan throughout the patient stay, including discharge preparation, planning, and transition to outpatient Home Infusion services.
  9. Collaborates with Case Management, Medical, Nursing, Ancillary and Home Infusion staff to eliminate barriers to efficient delivery of care.
  10. Ensures that all elements critical to the Nutrition plan of care has been communicated to the patient/family or receiving facility, in collaboration with the Interdisciplinary team.
  11. Coordinate the assessment, nutrition care plan, education and supplies for Nutrition Support patients outside of acute care requiring Home Infusion services.
  12. Utilize community resources and programs to assist patients in their plan of care.
  13. Continuously evaluates and monitors the effectiveness and outcomes of medical nutrition therapy intervention. Fosters continuity of care across the continuum of care, including discharge planning
  14. May include outpatient nutrition counseling.
  15. May teach Cardiac and Pulmonary Rehab classes, community education classes, and staff education.
  16. Complete chart notes per organizational policy.
  17. Ability to function independently and as a member of a cross-functional team. Able to organize and set work priorities in a multi-task environment.
  18. Participates in LEAN performance improvement and research activities.
  19. Required to maintain active Basic Life Support (CPR) certification.
  20. Other duties as assigned.

Qualifications

Required:

  • Master's Degree in Food and Nutrition, or related field or Bachelor's Degree in Food and Nutrition of related field with Internship completed before 12/31/2023.

  • 2 years of outpatient counseling and/or nutrition support experience.
  • Must be proficient with Windows navigation, Microsoft Office Suite.
  • Registered Dietitian with the Commission of Dietetic Registration.
  • Washington State Certified Dietitian (CD).
  • Maintain active Basic Life Support (CPR) certification and Washington State Food Handlers Permit.
  • WA DOH Medical Assistant Registered License.

Desired:

  • Master’s degree.
  • Electronic Medical Records experience.
  • Must be proficient with Windows navigation, Microsoft Office Suite.
  • Advanced Practice/Board Certification in relevant area of expertise.
  • AND or ASPEN registration.
